I need to write a new customer sign up screen. I'd like to use something that customers are familiar with. Is there any dominant user interface design for new user functionality? I looked at a few apps like Evernote but didn't see any consensus

I can't recall ever signing up for an account directly in an app -- usually, I do it from the Web site
